Having a birth plan can help you articulate your preferences and make informed decisions. Let's walk through a birth plan crafted by a labor and delivery nurse, which blends practical insights and real-world experience. Remember, this is your birth experience, and our goal is to empower you with the information you need to align with your desires and the best medical practices available.

Don't worry, this is meant to be informative - let's ensure that we're on the same page:

  • IV Access: An IV is a standard practice and can be important for fluid administration and medication if needed. I promise to make this as quick and painless as possible. If you’re okay with it, it ensures we're ready for anything.
  • Fetal Monitoring: Monitoring your baby's heartbeat is key to ensuring everything is going smoothly. If you'd like to move around, portable monitors could be an option, keeping you comfortable while maintaining optimal monitoring. In case intervention is needed, we'll keep you well-informed.
  • Artificial Rupture of Membranes (AROM): If there's a need for AROM to help labor progress, we're prepared. It's all about maintaining cleanliness, so I'll ensure plenty of fresh sheets and pads are available. Mobility is encouraged, and if wearing mesh underwear and peripads helps, we're all for it.
  • Labor Augmentation: If labor doesn't progress, I'm here to discuss options with you, including AROM, gentle nipple stimulation to encourage contractions, or starting Pitocin, should it become necessary. Let's give your body the time it needs to respond naturally.
  • Physical Exams: These are vital for gauging progress. I'll perform them as needed, working with you to maintain your comfort while ensuring clinical efficacy.
  • Catheterization: If an epidural limits your ability to feel the urge to urinate, we'll use a catheter. To preserve dignity and comfort, a Foley can be placed unless we’re close to delivery, then a quick cath can do the job.
  • Pain Management: I support your desire to try natural childbirth but know that every labor is unique. We have tools like a shower, a birthing ball (disinfected, of course), or an epidural if required. You can voice changes in your preference anytime – I'm here to assist.
  • Students/Residents/Attendings: Your comfort with the medical team is paramount. Feel free to voice who you're comfortable with in the room. Clear, professional communication is my promise, and you'll be at the center of that conversation.
  • Delivery Method: A vaginal delivery is typically preferred, and we'll encourage the best positioning and procedures to make it possible. Should the need arise for instrumental assistance (vacuum or forceps), or a cesarean, rest assured, we will discuss each step and make informed decisions with you.
  • Cesarean Section: If it becomes medically necessary, your comfort and safety remain our top priority. We'll ensure effective anesthesia and a gentle, careful procedure – no shortcuts.
  • Episiotomy: We'll try to avoid this unless absolutely crucial. Local anesthesia can always be administered, helping maintain comfort.
  • Personal Hygiene: These aspects don't hold medical importance today, but personal comfort does. We'll maintain a clean environment respecting your choices about hygiene.
  • Immediate Post-Delivery: After your baby's born, you'll get to hold them if they’re stable. Sharing this bonding moment and allowing your partner to cut the cord (timing at your discretion) are among our priorities. Your baby's well-being will guide immediate decisions.
  • Cord Blood Donation: If interested, we'll facilitate public donation when available.
  • Nutritional Intake: While typically advised to avoid solid food during active labor, we focus on your comfort and understanding of any choices you make. The priority is a healthy mom and baby, and we're thrilled to support this milestone in your life.
  • Newborn Care: We provide Vitamin K and erythromycin as standard practices for your newborn's health, and we’ll support your breastfeeding journey with advice and encouragement. If circumcision is elected for a boy, our skilled team will ensure precision and care.
  • Family and Visitor Management: Your labor space is a sanctuary for you and the partner you choose to have by your side. We respect your preferences for visitors and advocate on your behalf to ensure your peace and comfort. I'm excited to be a part of this pivotal time. Trusting each step in your birth plan to reflect informed decisions and aspirations is crucial, and my role is to support you every step of the way as you welcome your little one into the world.
